Residents in our Supported Living program live in local apartments where staff help with the activities of everyday life.  Residents gain confidence by meeting goals and working towards becoming more self-sufficient. The program provides protective oversight while also giving individuals the autonomy for independent decision making, community integration, and life skills development.

Supported Living

Group Home/Semi-Independent Living

Adults with I/DD who have a comprehensive waiver and have been approved for residential services.

Individuals’ homes within one of our four 8-plex apartment buildings and community locations.

Service available 24 hours a day/ 7 days per week.

Daily but varies based on the needs identified in the individual’s plan.

Department of Mental Health – DD Home and Community-Based Medicaid Waivers, SSDI

A benefits wage assessment is completed and any individual resources are included to be used for living expenses

Individuals are placed on a consumer referral database through the Department of Mental Health.  Supported Living administration monitor the referral database when there is an opening to initiate communication with potential residential candidates.

No, communication occurs in person and via the use of the phone.  However, Telehealth services via StationMD are used for medical questions and guidance for staff and residents if needed.
