Community Resource Program - Program Description

What is the Community Resource Program with Life and Work Connections?

The following information is shared to understand what our services have to offer and to help you determine whether it will meet your needs.

 

Every individual served by this program is assigned a Community Resource Specialist. Community Resource Specialists:

  • facilitate development of a needs assessment to address the individual’s and family’s unique support needs and preferences,
  • connect the individual and/or family to a variety of resources, including community resources and purchased services funded by the agency,
  • and monitor and assess the quality, timeliness and effectiveness of services and supports.

Resource Coordination

Boone County residents with a qualifying developmental disability, as defined by the Department of Mental Health, who do not have TCM eligible Medicaid are eligible for resource coordination services.

Agency office or virtual platforms

Services are provided based on preferences and availability of individuals and their families. Office hours are M-F, 8 am – 5 pm. Community Resource Specialists are available outside of office hours through the after-hours response system or prescheduled meetings with individuals and families.

The frequency varies based on individual needs and preferences.

BCFR

There are no fees. This is an agency provided service.

Anyone can complete a referral. Referral sources include:

  • public schools- including teachers, counselors, First Steps (early intervention) and early childhood programs
  • family members
  • therapists
  • physicians
  • state agencies, such as Children’s Division, Health & Senior Services, Vocational Rehabilitation

Services are provided directly to qualifying individuals by referring individuals to other community services.

Services may be delivered to individuals in Boone County or individuals transferring to Boone County via phone call, email, text, or video conference platforms such as MS Teams or Zoom
