What is the Family and Community Living Support Program?
The following information is shared to understand what our services have to offer and to help you determine whether it will meet your needs.

Each individual served is assigned a Coordinator, who provides:
- Planning: Coordinators facilitate development of an Individual Plan to address the individual’s and family’s unique support needs and preferences.
- Linking: Coordinators connect the individual and/or family to a variety of resources, including choices in community resources, purchased services funded by the agency and/or services available through a Home & Community Based Waiver.
- Monitoring: Coordinators monitor and assess the quality, timeliness and effectiveness of services and supports.

Family-based/Shared Living Supports – Family Services
Targeted Case Management (TCM)
Boone County residents with a developmental disability living in the community or receiving DMH funded residential or other services from identified providers are eligible for Service Coordination services. BCFR provides resource coordination services to individuals not currently enrolled in TCM eligible Medicaid. BCFR does not provide TCM to individuals receiving supported living services from BCFR.
Typically individual homes, agency offices or other community location of the individual’s preference.
Services are provided based on preferences and availability of individuals and their family. Office hours are M-F, 8 am – 5 pm. Coordinators are available outside of office hours through the after-hours response system or prescheduled meetings with individuals and families.
The frequency varies based on needs identified in the Individual Plan and individual preferences. Considerations include service monitoring requirements established as part of the Medicaid Home & Community based services the individual receives.
Medicaid for TCM eligible persons, BCFR, Department of Mental Health
There are no fees.
Anyone can complete a referral. Referral sources include:
- public schools- including teachers, counselors, First Steps (early intervention) and early childhood programs
- family members
- therapists
- physicians
- state agencies, such as Children’s Division, Health & Senior Services, Vocational Rehabilitation

Targeted Case Management services are contracted through the Department of Mental Health. BCFR then provides referrals to individuals for other direct services.
Services may be delivered to individuals in Boone County or individuals transferring to Boone County via phone call, email, text, or video conference platforms such as MS Teams or Zoom
