Support Coordination

We use a person-centered approach with a goal of helping individuals to THRIVE in the community, CONNECT with others, and ACHIEVE their personal goals.

Boone County Family Resources (BCFR) Support Coordination involves working collaboratively with individuals, families and other professional team members to develop and implement an individualized plan. We connect individuals and families with resources and services in the community to help meet identified needs. Once services are in place, we assist with monitoring the effectiveness of those supports.
