Participants in our Supported Living program live in local apartments where staff help with the activities of everyday life. With our support, residents gain confidence by meeting goals and becoming more self-sufficient. The program gives those who need protection and supervision the opportunity to be a part of the community.
Supported Living
Group Home/Semi-Independent Living
Boone County residents with a developmental disability who meet program admission criteria.
Individuals’ homes within one of our four 8-plex apartment buildings and community locations.
Service available 24 hours a day/ 7 days per week.
Daily but varies based on the needs identified in the individual’s plan.
Department of Mental Health – DD Home and Community-Based Medicaid Waivers
A benefits wage assessment is completed and any individual resources are included to be used for living expenses
Targeted Case Management providers make referrals based on expressed needs of individuals/families. Individuals are placed on a consumer referral database through the Department of Mental Health.
