To receive support through the Community Resource program an individual must be a resident of Boone County who has been determined eligible by DMH-DDD. Once determined eligible, individuals will be assigned to a Community Resource Specialist (CRS) who will follow up on their support needs.
Program participants will be individuals determined eligible for services by the Department of Mental Health – Division of Developmental Disabilities (DMH-DDD), who do not have Targeted Case Management (TCM)-eligible Medicaid and do not have an Autism diagnosis. These individuals are classified as Tier IV by DMH-DDD.
CRSs will serve all newly referred individuals in Tier IV with the very small exception of children who currently have siblings under age 18 served by Family Support; we made this exception to keep family services consistent for children under age 18. The CRSs will, in a scheduled and periodic way, accept the transfer of Tier IV individuals over age 18 from our Family Support program. Additionally, we will establish a process for transferring eligible individuals to the Family Support program including when an individual obtains TCM-eligible Medicaid, receives an Autism diagnosis, or turns age 15 for purposes of planning for transition from school to adult services and supports. Once an individual turns 18 or completes high school, whichever comes later, they will transfer back to the Community Resource program if they do not receive TCM-eligible Medicaid as an adult.
Supports through the Community Resource program are provided through Missouri Senate Bill 40 (SB40) funding, and there is no fee for participants.
