Individually Planned Services & Supports

The agency purchases and provides individually planned support services.  From newborns to the elderly, service plans vary greatly, but all are designed to connect individuals with developmental disabilities to the supports they need.  Agency Support Coordinators help eligible persons plan, locate, refer and coordinate services with other community agencies.  We authorize individually planned supports and services to be purchased by the County Board and the Department of Mental Health and Medicaid Home and Community based Waiver.  Through the Individual Plans we are also able to authorize services to be purchased by other funding sources.
